In cataract surgery, anesthetic methods have evolved from retrobulbal or peribulbal blocks to topical and intracameral anesthesia. However, some patients are allergic to anesthetics such as lidocaine, making it necessary to have alternatives for this group. We present the case of an elderly female patient diagnosed with bilateral senile cataracts who had experienced lidocaine allergy and anxiety that limited her ability to decide on surgery. A simultaneous bilateral cryophacoemulsification was performed under hypnosis in the microsurgery department of the Ramón Pando Ferrer Cuban Institute of Ophthalmology. The surgery was uncomplicated, with the patient fully cooperating and experiencing no pain. Along with constant psychological support throughout the procedure, the patient achieved immediate visual recovery postoperatively and was discharged four weeks after surgery.
