Correction of Involutive Eyelid Ptosis by Blepharoplasty with CO2 Ablative Laser

Abstract

Objective: To evaluate the results of surgical treatment for the correction of involutional ptosis using blepharoplasty surgery.

Methods: A descriptive, longitudinal, and prospective study was conducted in patients with a surgical indication for correction of involutional ptosis by reinserting the aponeurosis of the levator palpebrae superioris muscle to the tarsus through upper eyelid blepharoplasty. The procedure used a CO2 ablative laser as the cutting instrument. Patients attended the oculoplastics clinic at the "Hermanos Ameijeiras" Clinical Surgical Hospital between March 2023 and March 2025.

Results: The predominant age group was 72 to 83 years. 73,5 % were female, and 43,1 % presented with moderate ptosis. The most frequent intraoperative complication was bleeding, while postoperative complications occurred in 21,3 %, with persistent fat deposits being the most common (seven eyes). Asymmetry of the brow crease was present in only nine patients (10,1 %).

Conclusions: Women with moderate ptosis predominated. In most cases, good results were obtained with the surgical treatment of involutional ptosis using ablative CO2 laser blepharoplasty.
