Palpebral lacerations are common and challenging injuries for the ophthalmologist. We present a case of superior and inferior palpebral laceration repair using precise reconstructive techniques. A 43-year-old male patient with a history of hypertension was referred for trauma to the left eye with a stone involving the upper and lower eyelids. There were full thickness lacerations in the upper (10% of length) and lower (30% of length) eyelids without canalicular section. Surgical repair was performed under local anesthesia using 6-0 Vicryl. Reconstructive principles and techniques were applied, prioritizing precision in tarsal apposition and alignment of palpebral margins. Recommendations of several authors were followed to achieve optimal closure and avoid complications. No immediate complications were observed. Repair of full-thickness palpebral lacerations using precise and absorbable techniques can achieve satisfactory functional and esthetic results. Anatomic knowledge and application of key reconstructive principles are essential.
