Pediatric endogenous endophthalmitis is a rare intraocular infection, its diagnosis is “challenging”, the visual prognosis is poor, if not diagnosed and treated in time it can end in ptosis or evisceration of the eye. We present a patient who was admitted to the intensive care unit of the Pediatric Hospital of Centro Habana, in order to report the medical finding of endogenous endophthalmitis caused by bacterial pneumonia. The 8-month-old patient was brought for consultation due to an increase in volume of the right eye, and the presence of leukocoria, which is the initial manifestation of a wide spectrum of intraocular and systemic diseases, which is considered an ophthalmologic emergency. Its timely diagnosis and treatment is crucial in the correct visual development of children, therefore, irreversible amblyopia can be conditioned in this period. A quick detection of leukocoria and early assessment can prevent blindness in pediatric patients.
