Solitary Osseous Plasmacytoma of the Orbit

Abstract

Solitary orbital bone plasmacytoma, in the absence of multiple myeloma, is very rare. We present the case of a 60-year-old male patient who came for consultation because he noticed a painful and progressive left ocular protrusion since 3 months ago. A computed tomography was performed, which revealed an extensive ipsilateral sphenoidal osteolysis, with a slight increase in thickness and density of the neighboring soft tissue, which caused the proptosis. A possible primary neoplasm at another level was not demonstrated; while the absence of other bone lesions, anemia, Bence Jones protein in urine, bone marrow involvement, hypercalcemia and renal failure, together with normal immunoglobulin levels, excluded multiple myeloma. Therefore, the most probable diagnosis was considered to be that of a solitary bone plasmacytoma of the orbit. Subtotal resection of the lesion was performed and the diagnosis was confirmed by histopathologic examination.
